Please note: If you are assisting someone with setting up their account, please ensure that you are using the patient’s information when creating the account and not your own. We will be unable to connect the account if the information provided in the outside connection request does not match the patients record.

If you currently have an account that you created previously and want to send a connection request to Western Reserve Hospital, once you are logged into your old account, please follow the instructions below. If you need to create an account, please skip to the section below that tells you how to create an account. If you already have an account, but need to reset your password, please follow the directions towards the end of this information for the steps to reset your password. 

If your email was provided during registration, you should have received an invite that will allow you to connect your account with Western Reserve Hospital. If you are unable to locate the email and need further assistance on setting up an account or already have an account from a previous provider(University Hospital for instance), please follow the instructions below.

If you need to create an account, please skip to the section below that tells you how to create an account. If you already have an account, but need to reset your password, please follow the directions at the bottom of this document for the steps to reset your password.

If you are on a Mobile device and not a computer, please complete the steps under the Mobile Device section.


If you already have an existing account

1. Click on "My Account" at the top

2. Click "Connections"

3. Click Add Organization

4. Click Organization Type and choose "Hospital"

5. Type in the zip code of the hospital '44223'

6. Click Western Reserve Hospital

7. Click Connect

8. Click Accept

Once this has been completed it will send a request to our team and after verification, we will be able to connect your account.


If you DO NOT currently have a FollowMyHealth Account, please use the instructions below to create an account and request a connection with the hospital.

If you would like to create an account and request a connection, please follow the instructions listed below.

Please follow the instructions listed below so that you can request a connection to be sent to Western Reserve Hospital to connect your patient account with our patient portal.

1. Click HERE

2. Click on “I need to sign up”

4. Click I Accept

5. Create a username/password (Username does NOT have to be your email if you do not want) – If it says your email is already in use, then that means you already have an account from a previous institution (University Hospital more than likely) and you will need to reset your password (Please see below on how to reset your password.

6. Click Connect next to Western Reserve Hospital and then click Next

7. Click Next

8. Click Continue

Once you click Continue, then WRH will receive the request and accept it after verification has been completed.


If you need to reset your password, please follow the steps below

1. Click Forgot Password

2. Enter your username (more than likely your email address), click the captcha box and then click Continue

3. A verification code will be sent to your email address, enter the verification code and then click Continue (Do not press enter or it will send you another code)

4 Enter your new password and verify it and click Continue

5. Once the password is reset, please follow the instructions above to send the connection request to the Western Reserve team.


Mobile Device (iPhone, Android)

1. After signing up for an account you will be presented with a screen like the one listed below.

5. Click on the plus sign in the upper right corner

6. Type in 44223 as the zip code, click on Hospital and then click on Western Reserve Hospital

7. Click I Accept and wait for Western Reserve Hospital to approve the connection request.
