Western Reserve Hospital Recognized as Anticoagulation Center of Excellence

Western Reserve Hospital recently passed the Anticoagulation Forum’s assessment test and is now recognized as an Anticoagulation Center of Excellence. This illustrates Western Reserve Hospital’s strong commitment to providing the highest level of care to patients taking antithrombotic medications.

Courtney Hochman, PharmD, BCPS, Clinical Pharmacist at Western Reserve Hospital, passed the Certified Anticoagulation Care Provider exam, which qualified Western Reserve Hospital to become an Anticoagulation Center of Excellence for both inpatient care and the Anticoagulation Clinic.

“We are honored to have passed the Anticoagulation Centers of Excellence assessment,” said Dr. Robert Kent, President and CEO of Western Reserve Hospital. “The assessment and educational resources provided helped us streamline our operational performance, improve transition of care and knowledge of drug therapy and enhance patient education while learning more about disease state management.”

The Anticoagulation Centers of Excellence program helps healthcare professionals provide the highest level of care and achieve the best possible outcomes for patients on antithrombotic medications. The program offers a roadmap to consistent, sustainable excellence in key clinical and operational aspects of patient care.  Because it successfully met the rigorous standards in each patient care pillar, Western Reserve Hospital qualifies as an Anticoagulation Center of Excellence for 3 years.

Until now, there has not been a comprehensive standard to define ideal practices for an anticoagulation service or a central place to find relevant resources for our practice. This educational program will have a lasting and profound effect on anticoagulation care.

The Western Reserve Hospital Anticoagulation Clinic is designed to serve patients who are prescribed anticoagulation medications on an outpatient basis. 

The Anticoagulation Forum is a multidisciplinary non-profit organization that works to improve the quality of care for patients taking antithrombotic medications. It promotes the clinical application of evidence-based practices and provides education and networking opportunities for healthcare professionals.
